But being a parent can be a heavy load.
A social worker who specializes in maternal and child health can help parents be the best parents they can be! Under the direction of the Healthy Starts Program Manager, this position provides direct services to Healthy Starts Health clients and families in the greater Keene area.
As a member of a health care team, the Social Worker assists in developing and implementing an appropriate plan of care to meet client/family needs as well as education, support, and access to all available social net services.
This is a full-time (30 - 37.
5 hours/week) position which offers insurance benefits, vacation time accrual, sick time accrual and participation in the agency retirement plan.
Responsibilities include but are not limited to:
Assesses and manages social needs of clients through home visits and/or office visits; Is aware of role as agency representative in the community and therefore fosters good community relations through attainment of client satisfaction; Participates in interdisciplinary meetings and staff conferences to facilitate meeting the client's needs from admission through discharge; Conveys a positive image of the agency through personal appearance, attitudes and behaviors; and Knows and uses community resources, understands eligibility requirements.
Qualifications:
Bachelor's Degree in Social Work or Human Services; and Two (2) years of experience preferably in public health or human services; or A combination of education and experience that is deemed to be equivalent to the above noted educational and experience requirements.
Valid driver's license and access to an automobile during working hours.
